Orignal Description:During efforts to align sections of a pipe aboard the mentioned vessel, a control valve was mistakenly left open, resulting in a minor release of fuel into a waterway. On-site personnel intervened by shutting off the valve, stopping the release. There were no fires or injuries as a result of this incident. Plans for a cleanup operation are currently underway.

Human Error
SITUATION UPDATE:
Per Reporting Party: They wanted to update the substance being reported which is now Oily Bilgewater and not diesel fuel. OSRO, USCG, and QI responded, all of which found no recoverable oil. Cal DFW SOSC representative updated accordingly.
